Green Flags and Red Flags Tracker

A private, dated record of moments with your supports. Green flags are things going well. Red flags are things that concern you. Written down when they happen, in your own words.

🔒 Your record stays in this browser, on this device. Nothing is uploaded.

Please read: This is a record-keeping tool, not a rating system and not advice. It records what you write, exactly as you write it. It doesn't judge providers or tell you what to do. If you decide to raise a concern, a dated record like this makes the conversation much easier.

If a red flag is about safety

If you are in danger, call 000. If a worker or provider has been violent, abusive, or neglectful, you can report it to the NDIS Commission on 1800 035 544. You can stay anonymous if you prefer. Module 7 and Module 8 of the rights course explain your options.

What each one is useful for

Green flags help you notice which supports actually work for you. They're useful when renewing a service agreement, or when telling a new provider how you like support to be given.

Red flags help you spot patterns early. One late shift is a day. Five in a month is a pattern. If you ever make a complaint, your dated notes become your timeline.
